To reserve your spot you must complete the following:

1. Fill out the form below & prepay

2. You can pre-pay either by calling 1.800.466.1152 and asking for Colette or waiting for a customer representative to call you for payment after you submit the form.

3. Once we receive payment we will email you a copy of your receipt which will confirm your spot.

4. We will need your teammate's names by this date July 25th 2025.
